About Grace Medical Center

Grace Medical Center, formerly Bon Secours Hospital, has provided healthcare services to the West Baltimore community for more than 100 years. As part of LifeBridge Health’s commitment to this mission, we continue to provide the highest level of care regardless of socioeconomic status or ability to pay.

 

Grace Medical Center was acquired by LifeBridge Health in 2019. Since then, the freestanding medical facility has become a one-stop-shop for primary and specialty care. Our team serves patients of all ages by removing barriers to care, offering personalized health plans and encouraging health literacy.

Primary Care

The Family Health and Wellness Center, located on the third floor of Grace Medical Center, is fully equipped to treat common illnesses, address minor injuries and perform routine health exams either in-person or online. Primary care pediatric services are also offered at this location and throughout the community via mobile van.

 

Our on-site care coordination team ensures the convenience and consistency of all care. Patients and providers have easy access to this team, which offers support services such as:

 

  • Patient advocacy and education
  • Personalized care plans
  • Appointment scheduling
  • Transportation to and from appointments
  • Referrals and access to additional care
  • Quality assurance

Emergency Medicine

The emergency department at Grace Medical Center provides West Baltimore communities quality care with a brand new, state-of-the-art facility. Our 24-hour emergency department is ready for you when you need us most.

Substance Use Disorder Services

The New Hope Treatment Center at Grace Medical Center offers outpatient care for substance use disorders. Services include medication-assisted treatment with methadone and Suboxone, drug-free programs, peer recovery support, intensive outpatient care, therapy and DUI/DWI education.

Outpatient Mental Health Services

Grace Medical Center’s Mental Health Walk-In Clinic provides comprehensive outpatient mental health care for children and adults. Services include individual, group and family therapy; medication management; and intensive outpatient and partial hospitalization programs.

Community-Based Services

Our team brings healthcare beyond clinic walls with services including case management, mobile treatment, individual and group therapy, 24-hour crisis management and an adult psychiatric rehabilitation program (PRP).

Mobile Health

The LifeBridge Health Care Happens Here van brings care directly to schools, churches, neighborhood organizations and senior living communities. On-site health services include pediatric clinics, well-child visits, sports physicals, health screenings and general wellness support from our nurses.

Primary Care

Grace Medical Center’s Family Health and Wellness Center offers primary care for all ages. We treat common illnesses, minor injuries, physicals, screenings and vaccinations—in-person, online or via a mobile unit. Care coordination ensures access to referrals, transportation and support services.

Pediatric Care

From routine checkups to specialized care, Grace Medical Center helps children thrive. Services include well-child visits, sick visits, vaccinations and care for asthma, ADHD, cerebral palsy and developmental milestones, ensuring kids stay healthy at every stage.

Specialty Care

When advanced care is needed, Grace Medical Center connects patients with specialists in cardiology, gastroenterology, nephrology, orthopedics, OB/GYN and more. Located just steps from primary care, our Specialty Clinic ensures easy access to expert treatment.

Supporting Services

Our team offers advanced diagnostic and rehabilitation services to support your care. On-site options include vascular care, 3D mammography, imaging (X-ray, CT scan), pulmonary function testing, cardiac testing and outpatient physical therapy for seamless, coordinated treatment.

Renal Dialysis

Our Renal Dialysis Program provides life-sustaining treatments to patients who require renal dialysis to improve the effects of end-stage renal failure. Call 410-362-3094 for more information or to schedule an appointment. 
 

Walgreens Pharmacy

Grace Medical Center’s on-site Walgreens pharmacy is available to provide medications for patients, visitors and the surrounding community.
