At Grace Medical Center, we recognize the deep connection between faith, health, and community support. The Faith Community Partnership (FCP) Program is designed to empower faith-based communities by providing free health education, care coordination, and advocacy for congregation members. 

Through this unique partnership between churches, hospitals, and community organizations, we offer preventive health education, follow-up care support, and guidance in navigating the healthcare system—at no cost to you. 

How the Faith Community Partnership Program Supports You

Managing your health can feel overwhelming, but you don’t have to do it alone. The FCP Program provides:

  • Health Education and Prevention: Learn about managing chronic conditions, nutrition and wellness through health fairs and guest speakers. 
  • Care Coordination and Follow-Up Support: Get help navigating doctor’s visits, treatment plans and hospital follow-ups. 
  • Personalized Healthcare Navigation: Assistance in choosing the right doctor or clinic for your needs. 
  • Hospital Advocacy: Support and guidance if you need to seek emergency or specialty care.

How It Works

Each participating church appoints a liaison, who works closely with navigators from Grace Medical Center’s Community Health and Wellness team. Together, they: 

  • Provide health education and outreach to congregation members. 
  • Offer support and guidance during hospital stays. 
  • Assess individual health needs and connect members to appropriate care. 
  • Assist with hospital discharge planning and follow-up care. 

As part of LifeBridge Health, Grace Medical Center participates in CRISP (Chesapeake Regional Information System for Patients), a health information exchange that helps healthcare providers coordinate care efficiently.  

 

Who Can Join? 

  • The program is open to faith communities partnered with Grace Medical Center 
  • Participation is free and voluntary.  
  • You can opt out at any time by providing written notice.
