At Grace Medical Center, we know that where you live impacts your health. The Health & Housing Program is designed to provide free, personalized support for residents in our partner apartment communities, helping you stay healthy, access care and recover after a hospital visit. 

Our team connects you with health education, care coordination and advocacy to make navigating the healthcare system easier. Whether you need preventive health resources, help finding a doctor or follow-up care after an illness, we’re here to support you—at no cost to you. 

How the Health & Housing Program Helps You

Managing your health can feel overwhelming, but you don’t have to do it alone. Our program provides:

  • Health Education and Prevention: Learn about managing chronic conditions, nutrition and wellness through health fairs and guest speakers. 
  • Care Coordination and Follow-Up Support: Get help navigating doctor’s visits, treatment plans and hospital follow-ups. 
  • Personalized Healthcare Navigation: Assistance in choosing the right doctor or clinic for your needs. 
  • Hospital Advocacy: Support and guidance if you need to seek emergency or specialty care.

How It Works

Each participating building has a residential liaison, who works with a service coordinator and Health & Housing navigators—trained professionals from Grace Medical Center’s Community Health and Wellness team. Together, they: 

  • Assess your needs during a hospital stay. 
  • Help you transition home safely with a clear treatment plan. 
  • Connect you to local health resources and ongoing care. 

As part of LifeBridge Health, Grace Medical Center participates in CRISP (Chesapeake Regional Information System for Patients), a health information exchange that helps healthcare providers coordinate care efficiently.  

 

Who Can Join? 

  • The program is available exclusively to residents in partner apartment communities served by Grace Medical Center.  
  • Participation is free and voluntary.  
  • You can opt out at any time by providing written notice.

    Is this an insurance program?

    No. The Health & Housing Program is not insurance but a support network to help you access and understand healthcare services. For insurance-related questions, contact your insurance provider or LifeBridge Health’s business office. 

    Is this an insurance program?

    By participating in the Health & Housing Program, you allow our team to share basic details about your care to better assist you. This may include your name, general condition and hospital location. You may opt out at any time.

    What is CRISP, and can I opt out?

    Grace Medical Center is part of the Chesapeake Regional Information System for our Patients (CRISP), a health information exchange that helps providers coordinate care. You can opt out by calling 1-877-952-7477 or visiting www.crisphealth.org
