How do go about obtaining a copy of my birth certificate for myself or someone I know?

All birth certificates are issued by the Bureau of Vital Statistics. Sinai Hospital does not issue birth certificates. You can contact the Bureau of Vital Statistics at or you can call Maryland Vital Records at 410-764-3038.

I am coming out of town and will need a place to stay. Do you offer any accommodations or lodging?

Designed with the patient in mind, several guest suites are available at The Hackerman-Patz House at Sinai. Located just steps away from the main campus, the facility provides lodging for patients and families traveling to Sinai or other LifeBridge Health facility for appointments, rehabilitation or procedures requiring overnight or long-term stays. How do I obtain a copy of my medical records?

To obtain copies of your medical records, print and complete the "Authorization for Release of Medical Information" PDF form and mail it directly to the specific facility or office you want to request records from. We do not accept requests for medical records by e-mail. Please allow 7 to 10 business days for your request to be processed. When completing the authorization form, please provide all requested information and be specific regarding the dates of service and types of reports needed. If the records are being released directly to your private physician or another healthcare facility, there is no charge associated with copying the records. However, if you request that copies be released to yourself or others, photocopying charges will be applied. If you have a follow-up appointment, be sure to include the date of your appointment.
