The Diabetes and Nutrition Center at Northwest Hospital, recognized by the American Diabetes Association as meeting the national standards for diabetes self-management education* and support, serves people with diabetes and other medical conditions that can be improved through nutritional counseling, diet and lifestyle changes. Through our one-on-one evaluations and group classes, we can help you better manage your health.

In line with COVID-19 best practices, all visits are being done virtually (i.e., telemedicine) until further notice.

Group classes providing a comprehensive overview of diabetes

In addition to individual appointments, the Diabetes and Nutrition Center offers small, comprehensive virtual diabetes self-management group education classes. Physician referrals are needed to attend these classes, which cover the following content areas:

  • Diabetes pathology and treatment options
  • Healthy Eating
  • Physical Activity
  • Medication Usage
  • Glucose monitoring and pattern management
  • Preventing, detecting, and treating acute complications
  • Preventing, detecting, and treating long-term complications
  • Healthy Coping
  • Problem Solving
Diabetes Support Group

The Diabetes Nutrition Center at Northwest Hospital, in conjunction with the diabetes centers at Sinai and Carroll hospitals, sponsor the LifeBridge Health Diabetes Support Group. The purpose of the support group is to give people with diabetes and their support persons a place to share ideas and make connections.

This free group meets virtually the first Thursday of each month from 5-6 p.m. on Zoom. Topics vary and are subject to change (Zoom link is the same for each meeting).

Diabetes technology education

Our diabetes management education includes helping patients understand:

  • Continuous glucose monitoring systems
  • How to use insulin pens
  • Insulin pump therapy
  • Information on new medications for glycemic control
Scheduling an appointment

We accept many insurance plans. A referral from your primary care physician or endocrinologist is required.

To schedule an appointment:

  1. Call 410-601-9355 (select option #2, then option #4).

  2. Complete the Diabetes Self-Management Education/Training and Medical Nutrition Therapy Services order form prior to your visit. The form should be signed by your physician and faxed to 410-469-5108.
Appointments can be made once orders are received.
For your first visit, bring the following with you:

  • A doctor's referral and any forms requested by the clinic staff
  • Chronicle Diabetes Assessment Form (print and complete prior to your first visit)
  • A list of all medications you are currently taking, including over-the-counter medications
  • A copy of your most recent lab work
  • Medical records (if appropriate)
  • A glucose monitor (if appropriate)
Please arrive at least 15 minutes prior to your appointment time. You should also complete the Diabetes Self-Management Education/Training and Medical Nutrition Therapy Services order form prior to your visit. The form should be signed by your physician and faxed to 410-469-5108.

Directions
The Diabetes and Nutrition Center at Northwest Hospital is located in the outpatient services area. Please proceed to the main entrance and, after entering the hospital, turn left. Check in at the reception desk in the Sara & David S. Brown lobby.
