With dedicated support from our diabetes nurse practitioner, personalized treatment plans and remote blood sugar tracking, this program helps you control your blood sugar and prevent complications through regular, real-time adjustments.

Personalized Treatment Plans
Digital Blood Sugar Monitoring
Track your blood sugar levels remotely. We collect and use data from most continuous glucose monitoring (CGM) systems, including the FreeStyle Libre, Dexcom and Eversense. This allows you and your care team to make immediate adjustments as needed.

Your first virtual visit includes a thorough health review and baseline assessment. We’ll discuss your health history, lifestyle and goals.
- Customized Care Plan
Based on your consultation, we’ll design a treatment plan with regular monitoring and real-time adjustments.
- Ongoing Support and Monitoring
Remote monitoring and frequent virtual check-ins allow for adjustments to your treatment plan as needed. Our team remains accessible for questions or additional guidance.

How do I schedule an appointment?
You can call us at 667-239-2443 to set up a virtual or in-person appointment with our diabetes nurse practitioner. Walk-ins are not available.
Do I need a referral to make an appointment?
We’re a specialist office, but a referral is only needed if your insurance plan requires one. If you’re unsure, check with your insurance provider or call us—we're happy to help.
Will my insurance cover these services?
We accept most major insurance plans. To confirm coverage, please contact your provider or our office. If a referral to another specialist is needed, we can help coordinate that. Please allow 24 hours for processing. Referrals can be faxed directly to the specialist or picked up in person.
What if I was just diagnosed after a hospital or ED visit?
Our clinic regularly supports patients who are newly diagnosed after leaving the hospital or emergency department. We’ll walk you through the next steps and create a care plan that works for you.

How do I access my medical record or lab results?
With the LifeBridge Health Patient Portal, your health information is available 24 hours a day, seven days a week via your computer, tablet or smartphone.
How do I request a medication refill?
Contact your pharmacy first—they’ll coordinate with our office to process your request. To avoid running out, request a refill when you have at least a week’s supply left. You can also reach out to our office via the patient portal for prescription refill requests.
