When you leave a LifeBridge Health hospital or emergency department, you will receive an email or text invitation to our GetWell Loop portal. This link seamlessly connects you with virtual healthcare providers, ensuring a smooth transition from hospital to home.

You may qualify for remote patient monitoring services if you have recently been discharged from a LifeBridge Health facility with a chronic condition like hypertension, diabetes, congestive heart failure, COPD and more. Our secure platform is designed to cater exclusively to the unique needs of patients managing these chronic conditions, and it is accessible only to those patients who have received enrollment invitations from one of our nurses.
