Cardiac Rehabilitation

Cardiac rehabilitation at LifeBridge Health is designed to help improve cardiovascular well-being for those whose doctors have recommended a medically supervised program of cardiovascular care. The program most commonly aids in recovery after:

  • STEMI

  • NSTEMI

  • Coronary Angioplasty w/STENT

  • CABG

  • Angina Pectoris, Unspecified

  • Heart Failure

  • Heart Valve Replacement/Repair

  • Heart Transplant

  • Cardiomyopathy

  • Transcatheter Aortic Valve Replacement

The program helps educate you about your heart condition and teaches about the lifestyle changes necessary for a healthier and more fulfilling life. The rehabilitation process begins during hospitalization and continues throughout your lifetime. Learn more about our program’s process below.

Phase 1: Inpatient Cardiac Rehabilitation

Phase I is for patients receiving care at the LifeBridge Health Cardiovascular Institute (CVI). While in the hospital, patients will receive education from the nursing team and Cardiac Rehab Navigator. The CVI team provides education to the patient and their family regarding cardiovascular disease and its recovery process, gives personal encouragement, and explains discharge planning with an emphasis on how to modify risk factors and make lifestyle changes. Referral to Phase II of the Cardiac Rehabilitation Program at Northwest Hospital is automatically provided to ensure a seamless transition.

Phase 2: Outpatient Monitored Cardiac Rehabilitation

The second phase of cardiac rehab is a 12-week, 36 session, medically supervised and telemetry-monitored exercise program located at Northwest Hospital. Phase II cardiac rehab is designed to help with lifestyle changes and risk factor reduction. In addition to exercise, the program includes diet and nutrition counseling and referrals to smoke cessation programs. The program uses a multidisciplinary approach with team members who include a Board-Certified Cardiologist, a Cardiac Rehab Nurses, a Program Manager, Degreed Exercise Specialists, and Specially Trained staff.

Program Features

  • State-of-the-art group exercise area including large-screen TVs.

  • An individualized exercise program which incorporates the use of recumbent and upright bikes, seated and standing elliptical trainers, and a strength training area.

  • Educational presentations designed to answer questions about lifestyle changes, nutrition, and exercise.

  • Flexible morning and afternoon schedules with hour-long classes meeting Monday, Wednesday and Friday.

Joining the Program

We accept referrals from Cardiologists in and out of the LifeBridge Network. Prior to outpatient enrollment, patients should check their individual health insurance coverage, as some insurance companies may require pre-authorization.
