Mr. Carter had been in the Grace Medical Center emergency department for days when it became clear that he was ready to leave, but didn’t know where to go.  

 

Living with cognitive impairment, Mr. Carter struggled to recall details about his situation. He couldn’t explain where he would go or how he would get there. The one thing he could remember was the name of a close friend who lived out of state.

 

That was enough.

Grace’s Case Management team reached out to the friend, who confirmed he was willing to help care for Mr. Carter. From there, the focus shifted to making the transition as safe and smooth as possible. Case manager Jessica worked closely with the caregiver, talking through details and coordinating next steps so Mr. Carter wouldn’t have to navigate the process alone.  

 

As discharge and travel plans came together, Jessica found an airline that offered an attendant service for passengers with cognitive disabilities. And with only one early-morning flight available, she picked Mr. Carter up from the emergency department before dawn, brought him to the airport and stayed with him until he was settled and accompanied by the airline attendant. She remained in touch until he arrived safely and was reunited with his friend.  

 

For Mr. Carter and many others, leaving the emergency department isn’t the end of care. It’s a step made easier by people who connect patients to the resources they need next.
