• Speech Therapy: Our speech-language pathologists support stronger memory skills, clearer communication and safer swallowing through personalized exercises and strategies. 
• Skilled Nursing: We provide in-home nursing care for a wide range of medical needs, including wound care, medication management and long-term condition support.  
• Joint Replacement: Our experts guide you through surgery preparation and recovery with a program that reduces pain, supports healing and gets you moving safely. 
• Home Visits: All our services are available in-home, making it easier for you to heal in a familiar space while addressing real-life challenges. 
• Dementia Rehab: Our therapists assess thinking and memory skills, then create care plans that improve safety and independence while supporting caregivers. 
• Low Vision: We help patients manage vision loss with tips and training that improve safety, independence and confidence.
