Patient Information

OBGYN staffThank you for your interest in Sinai OB/GYN Associates. Below is some information that you will find helpful for your visit with us.

Scheduling Appointments

At Sinai OB/GYN Associates, our patients are seen by appointment only.

You can always schedule your appointment by calling the office location most convenient for you. When you make your appointment, please indicate the reason for the appointment so that we may allow an appropriate amount of time to provide you with the best care. When you come to your appointment, please bring:

  • Your completed forms
  • Current primary and/or secondary insurance cards
  • A photo ID
  • Any applicable co-pays

We also offer same-day appointments for urgent cases. Please call our office as early as possible to schedule a same-day appointment.

We will make every effort to see you at your scheduled appointment time; however, from time to time emergencies may occur and routine office visits may be delayed. If you need to cancel an appointment, please do so at least 48 hours in advance. We reserve the right to charge a fee for missed appointments.

Insurance and Billing

Sinai OB/GYN Associates accepts most insurance plans. Please call to confirm.

We will submit insurance claims for all office and surgical services. As a courtesy, we also file secondary insurance claims. You are responsible for:

  • Co-payments at the time of service
  • Deductibles or co-insurance
  • Services not covered by your insurance company

It is important for you to be aware of the requirements of your health insurance policy so that you save out-of-pocket expenses and frustration later. Our staff will do its best to help, but we cannot know everything about the hundreds of insurance plans available to our patients. If you have questions about your benefits, call your insurance company’s member services department or go to your employer’s human resources department.

Any charges for which the patient is responsible will be billed in a monthly statement. Although your insurance company may cover the charges, you will continue to receive statements on your account until all charges have been paid.

You may pay your bill by mail, over the phone or in person. We accept cash, personal checks, Visa and MasterCard.

Payment is due at the time of services, except for Medicare or Medicaid patients. If you have an outstanding balance and are unable to pay the total within 30 days of service, you may arrange to pay in monthly installments. Contact our office at 410-601-5530 to arrange a payment plan.

Referrals

Some insurance companies may require a referral/authorization from your primary care physician before you see a specialist or have testing. It is your responsibility to obtain the referral/authorization, if required for your visit. Please ask our staff for any paperwork that may be required. If you are given a written referral or order form, please bring it to your appointment.
