Vendor Portal

LifeBridge Health requires all new suppliers and service providers to meet certain qualifications. Qualification requirements are based on services provided and products sold and supported. The following are minimum requirements for all suppliers and service providers:

  • Completed Vendor Registration - Includes Federal Tax ID or Social Security Number

  • IntelliCentrics Certification
    • Criminal Background Check
    • Seasonal Flu Vaccine
    • Proof of Employer General Liability Coverage
    • Office of Inspector General Check (OIG)

  • Adherence to LifeBridge Health’s:
    • Vendor Management Policy
    • Vendor Code of Conduct
    • Value Analysis Policy

  • Financial Stability – Minimum 3 years in business

New suppliers and service providers are required to register on the Supply Chain Management portal.

Registration includes the completion of:

  • Vendor Registration Form
  • General business information
  • W-9
  • Diversity certification (if applicable)
  • Local business status (if applicable)
  • References

Registration does not guarantee a business opportunity, place your company on an approved vendor list, or obligate LifeBridge Health to solicit requests for bids, quotations or proposals. It does not guarantee business opportunity or obligate LifeBridge Health.

Registration gives LifeBridge Health decision makers access to your company’s information when conducting supplier searches so they may contact you regarding upcoming opportunities.
