Summary of Position: Responsible for providing professional nursing care to patients following all agency, state and federal regulations. Assists physicians and team members in coordinating patient care. Functions as primary clinical liaison between patients and physicians. Provides nursing services under the supervision of a Registered Nurse. Must communicate with RN case manager daily to ensure coordination of care is provided.
Essential Functions:
•
Provides general nursing care to patients
•
Observes, records, and reports patient’s condition and reaction to drugs and treatments to physicians and RN Case Manager
•
Educates patient/family about diagnostic procedures, medications, nutrition, and maintenance of health and wellness
•
Follow up on laboratory results, document and communicate to RN, physician and team members
•
Prepare complete and accurate clinical notes. Submit all clinical documentation in time-frame per agency policy
Education and Experience:
•
Current-valid Maryland Licensed Practical Nurse (LPN) license
•
Associates Degree, preferred
•
Current BLS for Healthcare provider certification
•
Minimum of two years nursing experience, preferably in a home care agency
•
1-3 years of acute care experience
