Summary of Position: Responsible for the coordination and supervision of all patient care provided within the agency. Under direction of the Senior Clinical Manager, utilizes leadership skills to coordinate patient care services, serves as patient advocate, facilitates and ensures communication/collaboration among all team members. Ensures that the clinical team is aware and compliant with all agency, state and federal regulations. Acts as a resource to clinical team.
Essential Functions:
•
Facilitates effective communication and positive rapport between staff, providers and referral sources. Reviews orders, laboratory reports and coordinates the dissemination of information to nurses and physicians to ensure follow-up is completed

Makes patient and personnel assignments to ensure coordination of patient care is completed. Communicates and coordinates with scheduling team to ensure schedules are appropriate and accurate
•
Assures the development, implementation and updates of the individualized plan of care for all patients to ensure that patient needs are continually assessed
•
Monitors utilization of services to ensure appropriate utilization of resources are identified
•
Ensures patient care is completed and documented in accordance with agency standards and justifies services rendered
•
Monitors performance of clinicians for all direct reports including contractual staff
•
Accountable for internal case conferences, case management, field staff evaluation and development. Responsible for clinical meetings and in-services as directed by the Senior Clinical Manager
•
Encourages staff development by providing coaching, mentoring and recognition of clinicians
•
Ensures competency of staff to include clinical co-visits as required, mandatory skilled competencies, mandatory annual competencies, and compliance with state and federal regulatory standards
•
Collaborates with outside companies to coordinate patient care
•
Constantly works to deliver efficient, high quality services to obtain excellent patient outcomes
•
Accountable for clinical performance of staff and identify areas for improvement and enact changes to achieve that improvement as appropriate
Education and Experience:
•
Registered Nurse with Bachelor’s Degree, preferred
•
Current valid Maryland Registered Nurse license
•
5-7 years varied clinical background with minimum of three (3) years acute care experience plus three (3) years of recent home health experience required
•
One (1) year Medicare certified home health managerial experience required
•
Comprehensive knowledge of OASIS and the Medicare Conditions of Participation for Home Health
•
Strong clinical assessment and case management skills
•
Current CPR-BLS for healthcare providers required
