Baltimore Limb Deformity Course

The 20th Annual Baltimore Limb Deformity Course will be held Friday, September 3 through Monday, September 6, 2010, at the Marriott Waterfront Hotel in Baltimore, Maryland. The Advanced Ponseti Treatment for Clubfoot pre-course will be held on Thursday, September 2. Two post-courses, Limb Lengthening: Staying Out of Trouble and Rehabilitation in Limb Lengthening, will be held on September 7. Please see below for more information.

Pre-Course: Advanced Ponseti Treatment for Clubfoot
Thursday, September 2, 2010
Chairs: John E. Herzenberg, MD, and Bradley M. Lamm, DPM

This one-day course is intended for orthopedic surgeons, podiatrists, and allied health professionals who treat pediatric clubfoot. Attendees should already have a basic working knowledge of the Ponseti technique and some experience in treating children with clubfoot. The goal of this course is to introduce the next level of understanding of the Ponseti technique and to explore new innovations. The course includes lectures by internationally recognized experts, live patient demonstrations, and a casting lab.

Topics to be covered include:

  • Ponseti technique for teratologic and neurogenic clubfoot
  • Outcome studies, including gait analysis and pedobarographs
  • Ponseti versus French method
  • Atypical feet and modified casting methods
  • Recognition and treatment of relapse
  • Innovations in bracing
  • Adjuncts to the Ponseti method (Botox, electrical stimulation)
  • New discoveries in clubfoot genetics
  • Modified Ponseti method for vertical/oblique talus
  • Decision making regarding surgical approaches
  • External fixation as an option for the failed or relapsed foot
  • Worldwide outreach for the Ponseti method

20th Annual Baltimore Limb Deformity Course
Friday, September 3 through Monday, September 6, 2010
Chair: John E. Herzenberg, MD
Directors: Shawn C. Standard, MD, Janet D. Conway, MD, and Bradley M. Lamm, DPM

This course has been the standard in teaching lower extremity alignment for the past two decades. It is intended for orthopedic surgeons, podiatrists, and allied health professionals who treat long bone deformities. No prior experience is required. Our goals are to introduce basic and advanced concepts in limb alignment, corrective osteotomies, and internal and external fixation for limb reconstruction, including lengthening and realignment. The course includes lectures, deformity planning sessions, hands-on saw bone exercises, and a cadaver lab. Participants will have an opportunity to present adult and pediatric cases to their peers.

Although this is the 20th annual course, we have instituted extensive modifications and updates. More than 60% of the content in the labs is new, and new topics also will be covered in the lectures. Topics to be covered include:

  • Tibial and femoral deformities (uniapical and multiapical)
  • Sagittal and oblique plane deformities
  • Rotational deformities
  • Foot deformities
  • Monolateral/circular external fixation
  • Gait implications for angular and rotational deformities
  • Methods to measure deformity on both plain film and Picture Archiving and Communication Systems (PACS)
  • Surgical techniques for lower extremity osteotomies, soft tissue releases, and flap coverage
  • Navigation for corrective osteotomies
  • Fixator-assisted plating

Post-Course: Limb Lengthening: Staying Out of Trouble
Tuesday, September 7, 2010
Chair: Shawn C. Standard, MD
Directors: John E. Herzenberg, MD, and Janet D. Conway, MD

This course is intended primarily for orthopedic surgeons. The goal is for the internationally-renowned faculty to use their vast limb lengthening experience to provide pearls, tips, and tricks in avoiding serious complications and handling difficult outcomes. The course includes lectures, case presentations, and interactive discussions.

Topics to be covered include:

  • Posttraumatic lengthening
  • Congenital lengthening
  • Intramedullary lengthening devices
  • Pin care problems
  • The unstable hip
  • The unstable knee
  • The stiff knee
  • The stiff ankle
  • Delayed bone healing
  • Infection

Post-Course: Rehabilitation in Limb Lengthening
Tuesday, September 7, 2010
Chair: Anil Bhave, PT

The course is designed for physical and occupational therapists, physician assistants, nurses, nurse practitioners, and orthopedic technologists. Rehabilitation can be problematic for patients undergoing limb lengthening with external or internal fixation. Course participants will be introduced to innovative therapeutic methods that were developed at the Rubin Institute for Advanced Orthopedics to successfully manage and avoid problems that occur during and after limb lengthening. The goals are to review pathophysiology and current management standards. Innovative treatment techniques (e.g., splinting, dynamic casting) will be discussed and demonstrated. Topics to be covered include:

  • Rehabilitation considerations for children and adults
  • Pin care
  • Techniques for managing stiff joints
  • Splinting strategies
  • Hydrotherapy (pool therapy)
  • Electrical stimulation
