Welcome to LifeBridge Health! We are proud and pleased to welcome you to our facilities. We hope to provide you with a dynamic and supportive learning environment that allows you to make the most of your clinical learning opportunity in our facilities.
Whether you are in our teaching hospital at Sinai, our award-winning community hospital at Northwest, or our wonderful and progressive long-term care facility at Levindale, we know that you will learn valuable skills and enjoy working with our knowledgeable and experienced staff. You will be using state-of-the art technology and see cutting-edge therapies while caring for patients and families with a wide variety of illnesses or conditions. You will have the opportunity to interact with staff and visitors from all walks of life and from countries around the globe.

For New Students: Before You Arrive
If you are about to begin a clinical rotation at Sinai Hospital, Northwest Hospital or Levindale, there are a number of things you need to be aware of. The first thing to do is review the below orientation manual (please read all pages and do not skip any material).
Instructors and students, it is required that you completely read through general information in the LifeBridge Health Nursing Student Orientation Manual. In addition, you are required to read information specific to each facility (Sinai, Northwest, and/or Levindale) in which you will be experiencing a clinical rotation.
Women and Children’s Services Rotation
at Sinai Hospital
Nursing students and instructors who have a clinical rotation on the Mother/Baby, Pediatrics, NICU, PICU and/or Labor & Delivery units must review the “Sinai Infant and Child Security – What You Need to Know” presentation and complete the post-test with each rotation.
Follow the link at the bottom of this page to access the presentation. Completion certificates for each student and instructor MUST be submitted to the Education Resource Center (ERC) office prior to the issuance of the Badge Form. Anyone who has not completed the post-test will not be allowed on the unit. PLEASE NOTE: Site coordinators/instructors who perform site visits for the practicum students must complete the security test as well.
Anyone entering Women’s and Children’s Services in the Blaustein Building must wear a LifeBridge Health ID. Name tags are not acceptable. Students or instructors not displaying ID will be asked to leave the unit. Students and instructors should not cover their photo ID badge with the badges given to them at the security desk. Your badge must be worn at shoulder level with the photo clearly visible.
Information for School Coordinators
Affiliation Agreements
LifeBridge Health uses a standardized affiliation agreement when working with school partners. The agreement outlines specific school and hospital rights and responsibilities, describes insurance and oversight expectations, and details required elements that must be in place for students to participate in clinical experiences at our sites (immunizations, safety and infection education, background checks, etc).
The LifeBridge Health agreement is “evergreen,” meaning that once fully completed the agreement will remain in place until one or both parties decide to terminate the relationship. Each LifeBridge Health site requires a separate agreement, as each is a separate entity within the LifeBridge Health system. Questions regarding content within the agreement can be directed to the respective coordinator at each clinical site.
Information on Training Modules
All student nurses, instructors and practicum students are required to take a computer class prior to starting clinical rotations if they are going to be documenting in the patient record or administering medications during the rotation.
This is an online course. Students and new instructors must complete the online modules and competency tests, and print the certificate of completion at the end. Students are to give the completion certificate to their clinical instructor. Instructors and students must complete the computer-based orientation to receive access to the electronic medical record.
Pre-clinical Rotation Checklist
Passport Packet
Clinical Coordinators must verify necessary documentation needed for each student and instructor by completing the Nursing Student Passport. This form is to verify that students and instructors have the necessary immunizations, drug screening, background check.

School Coordinators
For each clinical group, please send the following documents together in one email. Please make sure all forms are completely filled out, accurate and legible. Forms should be completed according to the LifeBridge Health Student Orientation Manual.
Passport Packet for each clinical group includes:
- Copy of each instructor’s and students’ LifeBridge ID – both employee and/or student badge if you have one
- Copy of each Student and Instructor’s Government Issued Photo ID – no student IDs accepted
- Orientation Manual Confirmation & Parking/Badging Registration Form for specific hospital were rotation is happening (one for every student and instructor)
**Please note this confirmation page, located in the orientation manual, needs to be printed and signed by each student and instructor for the clinical site.** - Certificate of completion of Cerner online program (one for every student and instructor that needs patient chart access)
- Certificate of completion for Blaustein Security Module (one for every student and instructor)
**This is only required if students will be rotating through Women’s and Children’s Services at Sinai.**
Email all information at least 15 business days (except holidays) before the start of clinical rotation to [email protected] and include the clinical site name in the subject line. If your paperwork is not received at least 15 days before the start of clinical rotation, clinical start will be delayed until the paperwork is received and approved.
Once each group’s paperwork is approved, your respective facility will be following its own processes for badging and starting clinical rotation. Please refer to the student orientation manual for each facility’s badging and clinical start processes.
Practicum Students- must follow the process as outlined above
All practicum students will be partnered with a preceptor and will need ERC approval. Once the student is paired with a preceptor, all required paperwork must be completed and turned in before starting any hours.
For Nursing Instructors
Are you facilitating a clinical or practicum student rotation? All instructors must attend an orientation session prior to bringing students onsite.
