LifeBridge Health Cardiac Rehabilitation Program

LifeBridge Health Cardiac Rehabilitation Program Cardiac Rehabilitation at LifeBridge Health is a three-phase program designed to help improve cardiovascular well-being for those whose doctors have recommended a medically supervised program of cardiovascular care. The program most commonly aids in recovery after a heart attack, angioplasty and/or stenting and open-heart surgery. It is also beneficial for patients who suffer from stable chest pain. The program is designed to educate you and your family about your particular heart problem, and help you make lifestyle changes necessary for a more satisfying and fulfilling life. The rehabilitation process begins during hospitalization and continues throughout your lifetime.

Phase I - Inpatient Cardiac Rehabilitation


Phase I is for patients receiving care at the Heart Center at Sinai and begins with a visit by the Cardiac Rehabilitation education program nurse. The nurse provides education to the patient regarding cardiovascular disease and its recovery process, gives personal encouragement and explains discharge planning with an emphasis on how to modify risk factors and make lifestyle changes. Referral to the LifeBridge Health Phase II Cardiac Rehabilitation outpatient program at Northwest Hospital is encouraged and arranged upon discharge.

Phase II - Outpatient Monitored Cardiac Rehabilitation


The second phase of Cardiac Rehab is a 12-week, medically supervised and telemetry-monitored exercise program located at Northwest Hospital. Phase II Cardiac Rehab is designed to help with lifestyle changes and risk factor reduction. In addition to exercise, the program includes diet and nutrition counseling, stress management, smoking cessation classes and education. The program uses a multidisciplinary approach with team members who include a board-certified cardiologist as the medical director, a Phase II Cardiac Rehab nurse, a RN as the program manager, a physical therapist as the program director, a registered dietitian and fitness staff.

LifeBridge Health Cardiac Rehabilitation Program Northwest Hospital's Phase II Cardiac Rehab Features:
  • New state-of-the-art private exercise area including large screen TVs and comfort station.
  • An individualized exercise program incorporating the area's largest array of high-tech equipment and facilities, including recumbent and upright bikes; stretching station; seated and standing eliptical trainers and rowing machines.
  • Educational programs designed to answer questions about lifestyle changes, nutrition and exercise.
  • Expert staff members including nurses, fitness technicians, a physical therapist, a board-certified cardiologist/medical director and medical supervision.
  • Onsite registered dietitian for nutritional counseling. 
  • Licensed psychologist available by referral.
  • Flexible schedule with hour-long classes meeting Monday, Wednesday and Fridays mornings at 7:00, 8:15, 9:30 and 10:30 and afternoons at 3:00 and 4:00.

LifeBridge Health Cardiac Rehabilitation Program

Phase III - Outpatient Unmonitored Cardiac Rehabilitation


The third phase of the program focuses on exercise that is unmonitored, or performed without a heart monitor. The Phase III program is for patients who have completed the Phase II program and still desire supervision of medical staff during exercise. The goal of Phase III is to promote long-term lifestyle changes and adhere to an appropriate exercise regime. Cardiac rehabilitation patients are referred to LifeBridge Health & Fitness for this phase of the program.

Joining the Program

The LifeBridge Health Cardiac Rehabilitation outpatient program at Northwest Hospital accepts referrals from physicians and other healthcare professionals. Prior to outpatient enrollment, patients should check their individual health insurance coverage, as your insurance company may require pre-authorization.

New patient orientation sessions are scheduled for 11:30 a.m. and 1:30 p.m. on Mondays, Wednesdays and Fridays. To schedule a new patient appointment, call 410-701-4333.
