Electronic Personal Health Record

Investing just a few minutes to create and maintain a personal health record (PHR) will ensure that you have comprehensive health information whenever it's needed – whether it's an unplanned trip to your local emergency room or a visit to a doctor while on vacation – wherever you have Internet access. As the owner of your PHR, you control the confidentiality of the data, who can access your PHR, and what information your doctors can and cannot see.

To register for your PHR, contact the LBH Call Center at 410-601-4PHR (4747). Your registration will be processed and you will receive an enrollment letter from Cerner Corporation, the medical data center that hosts MySELF. The letter will include your temporary security code and directions on how to access the site.

What is a Personal Health Record?

A Personal Health Record is a collection of healthcare related information about yourself. It can provide doctors with valuable information that can help improve the quality of care you receive. A PHR can help reduce or eliminate duplicate tests and allow you to receive faster, safer treatment and care in an emergency. In short, a PHR helps you play a more active role in your healthcare. What information is included in the MySELF Personal Health Record?
  • Demographic information

  • Medications

  • Surgeries / Procedures

  • Health Issues

  • General Lab Results

  • Immunizations

  • Allergies

  • Result Documents – ie electronic discharge instructions, Radiology exam reports (FUTURE), other textual result reports (FUTURE)

  • CCD – Continuity of Care Document – a summary of the most relevant data from your medical record from LifeBridge Health facilities. Provides a snapshot in time of specific medical information that can be printed by the user and taken to any non LifeBridge Health Provider.

  • FUTURE functionality – self-entered data such as new allergies since your last visit to LifeBridge Health, immunizations you received from non LifeBridge Health Providers, previous surgeries/procedures at non LifeBridge Health facilities, etc. Future functionality will also be available to allow users to either scan or download documents from files such as Living Will, etc.

  • Secure electronic messaging to your LifeBridge Health Provider. Templates for Prescription refills, Appointment requests, and general messaging are available from certain LifeBridge Health Physicians. Confirm your LifeBridge Health Physician's participation by checking the list of Physicians in any of the templates above within your PHR. If their name does not appear in the SEARCH when you enter either their last or first names, then the Physician is NOT participating at this time.

What information will be populated by LifeBridge Health?
All of the information included in the MySELF Personal Health Record is provided from documentation into their electronic medical record at our LifeBridge Health facilities – ie Sinai Hospital / Northwest Hospital, outpatient Laboratory / Radiology services.

Is there a fee for having a Personal Health Record?
No. There is no fee for enrolling or maintaining a Personal Health Record at LifeBridge Health. Your account remains active until you decide to opt out. Opting out will not close your account and ability to view old results that were in your account prior to your decision to opt out – but you will no longer be able to view new results based on any visits to LifeBridge Health.

Can I choose to open accounts for my dependents?
No – you yourself cannot open accounts for your dependents. However, each of them – if over the age of 18 years – may apply for their own MySELF PHR by completing the Request Form online


PRIVACY/SECURITY:
Who has access to my Personal Health Record?

As the owner of your PHR, you control who has access to your account. No one at LifeBridge Health has access to your account unless you provide it to them. Neither Cerner nor LifeBridge Health will remarket your information.

You alone control the information in your Account and are entirely responsible for the information in your Account. You are responsible for the security, confidentiality, and integrity of your user name and password and for any use of your Account, whether or not you authorize another person to access your Account. You should immediately notify the LifeBridge Health Call Center of any unauthorized use of your Account and we will work with you to ether change the security or close the Account.

LifeBridge Health will not access your Account unless requested or authorized by you. Even in the event you require medical treatment at LifeBridge Health, LifeBridge Health will not access any information in your Account to assist in your treatment but will need you to provide relevant information. You may print a copy of your account prior to visiting a LifeBridge Health physician or facility, or any other healthcare provider.

You may also provide "proxy" access to your account to whomever you choose – as long as that person is 18 years of age or older and has internet access. Follow the Proxy access process from within your MySELF PHR

Is my information secure?
As defined under the Health Insurance Portability and Accountability Act – (HIPAA)), LifeBridge Health takes privacy and security very seriously. In order to assist in meeting compliance with the HIPAA laws, both LifeBridge Health and Cerner have implemented administrative, physical and technical safeguards to secure health information. The MySELF Web site is safeguarded with a Secure Sockets Layer that transmits private application pages through the Internet. A state-of-the-art technology center in Kansas City, which maintains appropriate HIPAA policies and procedures, hosts the MySELF Health Record to ensure the physical security of the data. None of the participating companies gain any rights to data collected.

Why am I being asked to provide either a portion or entire Social Security Number if I decide to opt in?
Currently, claiming your MySELF account requires a Secret Key or PIN number. Cerner, the vendor supplying the PHR software, is using the last 4 digits of the SSN for this PIN number. In the future, the software will be modified to allow consumers to choose their PIN numbers.


Opt Out:
What if I don't want to no longer participate in the MySELF PHR?

Should you no longer wish to participate, you simply stop using the account. Your data remains safe in your LifeBridge Health electronic medical record.

If I opt out is all information about me permanently deleted from all databases, or is my account deactivated, but the information remains in a database?
The information in your MySELF PHR comes from the electronic documentation that your Physician does into your LifeBridge Health electronic medical record when you visit a LifeBridge Health facility. You are actually "viewing" into your electronic medical record. No data resides on any other database. When you opt out, you will no longer be able to "view into" your electronic medical record that is maintained by LifeBridge Health for any of your visits to our facilities.


ACCOUNT CLAIMING:
What is a Claim ticket?

A Claim ticket will be either emailed or direct mailed to you in response to your request to opt into the MySELF PHR. You will be asked to enter several pieces of information in order to claim your Account and begin entering/receiving data. Once you claim your Account, you can define your own ID/password into your Account.

Why did I receive a letter/e-mail from accounts@myhealtheexchange.com?
You received an email or letter from this address in response to your request to opt into the MySELF PHR. In order to begin using your MySELF account, you must first follow the instructions in this email / letter to claim your account.

Once I receive the account claiming information, what do I need to do?
Enter the website address or follow the web link that was provided in your registration letter. From that point, you will be asked a series of questions to verify your identity. Don't forget to have your claim ticket number available when you register. The process will take about five minutes to complete.

What happens if I do not initially activate my MySELF Personal Health Record?
Your MySELF opt in request will remain active until you claim your Account by entering in the Claim Ticket number, etc. that was included in the email / letter response to your request for opting in. You may claim your account at any future time as long as you save the Claim Ticket in order to enter the needed pieces of information. No one will be able to either claim your PHR or view your information without the specific Claim Ticket information that was emailed / direct mailed to you.


PHR CORRECTIONS:
Can I add or edit any information contained in my PHR?

Users cannot currently add or edit any information contained in the MySELF PHR. Future functionality will allow this. There is no date for the release of this functionality at this time.

I noticed the information populated into my PHR is incorrect. Is there anything that can be done to correct inaccurate information in our record?
If the information that has been uploaded by LifeBridge Health is found to be inaccurate, we can correct your MySELF account accordingly. Because your MySELF account is your information, we will need authorization to both research the clinical information here at LifeBridge Health and correct your PHR, if appropriate. Please call 410-601-WELL to begin the investigation process. We will notify you once the investigation / correction is complete.
