To Request an Appointment by Telephone
A patient service representative will ask you specific demographic questions and obtain information regarding your medical condition and insurance information. You will also be reminded to bring your medical records to the first appointment. These may include pathology and lab reports, x-rays and scans, slides or tissue blocks.
