What is Medication Therapy Management?
Medication Therapy Management involves a pharmacist working one-on-one with a patient to help them better understand their medications—including all prescription and over-the-counter medications, vitamins and herbal supplements.
Working together with the patient’s physician, the pharmacist seeks to identify any drug interactions or other issues that may arise with a particular set of medications. By identifying these problems and coming up with the ideal medication plan, Medication Therapy Management ensures that the best possible outcome is achieved for the patient.
Who can Medication Therapy Management help?
The program is ideal for patients who have 4 or more of the following conditions:
• Taking four or more medications on a long-term basis
• Being treated by more than one health care provider
• One or more chronic diseases that require medication
• Abnormal lab results for conditions treatable by medication
• Difficulty taking a medication according to its prescribed plan
• Taking too much or too little of a particular medication
• Being recently discharged from a hospital or nursing facility with new prescriptions
• Total medication bills of $200 or more per month
