LifeBridge Health and HomeCare Maryland Form Partnership to Enhance Care for Homebound Patients
LifeBridge Health and HomeCare Maryland have joined forces to provide health care for people in their own homes if they are disabled or living with one or more chronic conditions.
This new partnership offers a seamless method of continuing care for those in fragile health so they can make a smooth transition back home after a stay in a hospital or in a skilled nursing facility. The new HomeCare Maryland extends the reach of LifeBridge Health’s comprehensive services to make it convenient to people who need continuing care.

HomeCare Maryland provides a wide range of specialists who treat and care for patients in their own homes, according to instructions from their own physicians. These experts include nurses, physical and occupational therapists, speech pathologists, home health aides, social workers and dietitians.
