| Test Ordering: |
Test orders must be provided to the patient on a prescription with printed physician contact information, an electronic generated order or LifeBridge Health Outreach requisition signed by the physician. When using the Clinical or Cytology requisition forms, simply check the box provided and enter the Diagnosis Code (ICD 9) as medically indicated. For those tests not found on the form, simply write the test on any of the blank lines provided. Questions may be answered by calling 410.601.8735, Monday through Friday from 8:30 AM to 5:00 PM. Patients need to bring a photo ID and their Insurance Card to the Patient Care Center with the script or requisition.

| To submit a specimen for analysis: |
| 1. Requirements: |

If specimens are to be drawn in an office and submitted to LifeBridge Health Laboratory the following are required:
A completed Clinical Request Form or Cytology Request Form. The form must have complete patient demographic and clinical information as required to assure proper identification and processing. |

Federal Law requires the Physician signature. |

ICD 9 codes are required for each test requested. |

c)

Clearly identified tests on requisition. |

d)
|
To bill a patient insurance, a copy of the front and back of the insurance card must be provided or the office may submit a Face Sheet from the office medical record containing the patient insurance information. |

| 3. Specimen Pick-up Service: |
| |
LifeBridge Health Laboratory Services utilizes an in-house as well as a local commercial courier service. To dispatch a courier phone 410-601-8735 M-F 8:30 AM to 5:00 PM, or 410-601-5116 after hours. |

| 4. Specimen Delivery: |
| |
Specimens may be dropped-off at any of the Patient Service Center location during normal business hours or at Sinai Hospital, Department of Pathology, 3rd floor, 2401 W. Belvedere Avenue 24 /7. |

| 5. Supplies: |
| |
LifeBridge Health Laboratory Services will provide a specimen box, all forms and supplies to collect any biological specimen. To request items phone 410-601-8735 M-F 8:30 AM to 5:00 PM, or fax 410-601-8219. Please allow 72 hours for the processing and delivery of your order. |

| 7. Result Reporting: |
| |
LifeBridge Health Laboratory Services provides patient result reports to the physician office within 24 hours following receipt of specimens for most routine tests. Those tests requiring longer time to analyze are reported as soon as results are available. The computerized reporting system electronically transmits results once per day to physician offices that request this service. If results are not electronically transmitted they are mailed to the requesting physician office by US Postal Service. |
