LifeBridge Health’s post-acute services offer a continuum of care to help patients regain function and vitality after a life-altering illness or injury. Our highly trained multidisciplinary team members at Sinai and Northwest hospitals and Levindale Hebrew Geriatric Center and Hospital provide treatment plans for patients who require the following services:

  • Outpatient behavioral health
  • Long-term acute medical care
  • Long-term skilled nursing care

The first part of this recovery process is to understand the kind of care you need. Here are some basic explanations for the care we offer at LifeBridge Health:

An acute rehabilitation unit is a place where hospitalized patients receive three hours of daily therapy while being overseen by physicians who specialize in physical medicine and rehabilitation.

Subacute rehabilitation provides one to three hours of daily therapy for hospitalized patients, along with skilled nursing care.

Our Specialty Hospital provides a wide array of medical and rehabilitative services to patients who need the gift of time to help them recover from a very serious illness.

Our Brain Health Unit provides both medical and behavioral management of patients with chronic mental illness.
