How do I obtain a copy of my medical records?

To obtain copies of your medical records, you may print the Authorization for Release of Medical Information PDF form and mail it directly to the facility or specific office from which you are requesting records. We do not accept requests for medical records by email. Allow seven to 10 business days for your records to be copied. Please complete the authorization form, include all information requested and be specific as to dates of service or the types of reports needed. If the record is being released directly to your private physician or another health care facility, there is no charge associated with copying your records. If you have a follow-up appointment, be sure to include the date of your appointment. However, if you request copies to be released to yourself or others, there are photocopying charges. (State of Maryland fees for copying are $19.09 plus a per-page fee of $0.63 and the actual cost of shipping.)

How can I get clinical updates on my loved one's condition?

You can get an update by calling the unit directly and asking to speak to the nurse who is caring for your loved one.

Can I bring items such as food and clothing for my loved one? If so, how will they get the items?

You can have food and clothing items delivered directly to your loved one by leaving the items you've brought on the cart at the front entryway. Please be sure to put the patient's name and room number or unit on the items. A team member on the patient's unit will be contacted to come pick them up and take them to the patient.

  
How can I send "thank you" cards or notes to the staff?

We really appreciate your well wishes and thoughtfulness. You can personally drop off "thank you" cards and notes on the cart at the front entryway, or you can mail them to: 2434 W. Belvedere Ave., Baltimore, MD 21215. If you want to offer food, we can only accept food items that are pre-packaged or from a commercial food merchant; we cannot accept any homemade items.

How can I make a donation?

You can make a donation via our Care Bravely Compassion Fund website. The Care Bravely Compassion Fund was created specifically to enable those in the communities we serve to provide donations in support of our caregivers – the nurses, physicians and frontline staff who are working tirelessly to prevent the spread of the coronavirus (COVID-19) while providing quality care for patients. Your donations can help provide meals for staff, personal protective equipment (PPE) and other needs that may lie ahead. We are grateful for your generosity and support.
