What is an ACO?
An accountable Care Organization (ACO) refers to a group of doctors and other healthcare team members who agree to be responsible and accountable for both the quality and cost of the care delivered to a specific population of Medicare beneficiaries. This means that Medicare carefully measures and publicly reports on the quality of care delivered to our patients by our doctors, while also carefully monitoring costs in these patients. At LifeBridge Health we welcome this transparency and we are proud to have Medicare validate the results of our work.
How does the LifeBridge Health ACO affect my care?
The goal of Lifebridge Health ACO is to provide a continuum of efficient high quality services to its population though collaboration amongst its providers. A barrier to promoting collaboration and improving care coordination is the lack of information, which often leads to duplicate tests or having to answer the same questions over and over. By ensuring that your Primary Care Physicians have the most up-to-date information about your health, and the proper channels for communication with your specialists, we are committed to delivering high quality care and hope to provide you with a better care experience.
