The LifeBridge Health ACO strives to provide the highest quality services to our patients and their families, while also making their healthcare more convenient and more affordable.
By building a more complete healthcare team with the patient at its center (e.g. hospitals, physicians, care coordinators, health coaches, etc.), we successfully achieved a model of care that results in:
- Better health outcomes
- Better patient experience
- Higher quality of care
- Lower out of pocket costs for patients
What is an ACO?
An accountable Care Organization (ACO) refers to a group of doctors and other healthcare team members who agree to be responsible and accountable for both the quality and cost of the care delivered to a specific population of Medicare beneficiaries. This means that Medicare carefully measures and publicly reports on the quality of care delivered to our patients by our doctors, while also carefully monitoring costs in these patients. At LifeBridge Health we welcome this transparency and we are proud to have Medicare validate the results of our work.
