Medical
One choice of the LifeBridge Health, Triple Option Plan.

There is no deductible: select an in-network physician from the LifeBridge directory and pay a $10 co-pay and your insurance coverage is 100% for covered services. |

After meeting a $200 individual/$600 family deductible: select a physician from the BlueCross BlueShield PPO directory and your coverage is 80%/20% for covered services. |

After meeting a $200 individual/$600 family deductible: select a physician not affiliated with BlueCross BlueShield and your coverage is 60%/40% of allowed benefit for covered services. |
You have the option to waive medical coverage.
Prescription Drug
The prescription coverage is with Express Scripts. There is a $25 individual/$75 family deductible. There is a formulary list. Medications on the list that are generic have a $10 co-pay; medications on the list that are name brand have a $20 co-pay; and medications not on the list have a $35 co-pay. You may also utilize mail order and receive a 90-day supply of medication for two co-pays.

What's the Difference?
- You may choose to participate in the health care
spending account and/or the dependent care spending account. To take advantage
of their tax savings, decide how many pre-tax dollars you want to contribute
to each account every pay period. When you pay expenses with pre-tax dollars,
you increase your purchasing power by 25 - 30 percent.

What's the Plan?
- The ECP is a free core benefit provided by Sinai. If you need to talk to a professional about family, financial, job-related or other matters, call for confidential advice.
Term Life and Accidental Death and Dismemberment (AD&D)
Your Choices
- Free core benefit of $10,000 of life insurance and
$10,000 of AD&D
- Contributory life insurance and/or AD&D to one,
two, or three times pay, up to maximum
- Dependent life insurance for your spouse and
dependent children
- Waive dependent life insurance coverage
What's the Difference?
- The free core term life and AD&D benefit provides
you with $10,000 each of life and AD&D coverage. Your full life insurance
benefit will be paid to your beneficiary if you die while employed at Sinai.
If you die in an accident, your full AD&D benefits will also be paid to
your beneficiary. If you are injured in an accident, a percentage of your
AD&D benefit may be paid to you, based on the extent of your injury.
- Contributory term life and AD&D
coverage are also available. You may increase your benefit to one times your pay, up to
$200,000; two times your pay, up to $400,000; or three times your pay, up to $600,000. If you
choose more than $400,000 of coverage, or if you do not enroll when you are
first eligible, you must show evidence of insurability before coverage will
begin.
- Dependent term life insurance is available for any
eligible children and your spouse. You may cover your spouse with $10,000 and
your children with $5,000 each.
- You may waive dependent life insurance coverage.
Interest-Sensitive Whole Life

What's the Difference?
- Whole life insurance can cover you and your
dependents during and after your employment with Sinai. Your convenient
after-tax payroll deductions will be invested at competitive rates to earn
interest for you. Obtain an enrollment form from the Benefits Office if you
wish to participate.

Paid Leave
Vacation:
What's the Plan?
- Vacation must be used during the 12-month period of
service and cannot be accrued one year to the next.
- No more than two weeks may be taken consecutively.
- Requests must be discussed with the Chief Resident and made final by August 1 of each year. (If you are rotated through more than one department, approval is needed from the department to which you will be assigned during the requested vacation leave.)
Short and Long-Term Disability (STD and LTD)

- Free core STD benefit of 60 percent of pay, up to
$215/week
- Free core LTD benefit of $2,000/month
- Buy additional STD coverage and increase your benefit (60 percent of pay) to a maximum of $700/week).
What's the Difference?
- Free core STD benefits of 60 percent of pay, up to a
maximum of $215/week, are provided automatically at no cost to you. If you are
ill or injured, STD benefits will begin after you have used all your vacation
and sick leave, or after 30 days, whichever is later.
- Free core LTD benefits of $2,000/month are
provided upon application at no cost to you. If you are ill or
injured, LTD benefits will begin after you have used all your vacation and
sick leave, or after 90 days, whichever is later. This is a portable benefit
that can be continued on an individual basis after you leave Sinai.
- You may increase the maximum STD benefit to 60 percent of pay, up to $700/week.

Long-Term Care

What's the Difference?
- You may choose long-term care for you and/or your
spouse. This benefit provides coverage for extended care received for an
illness, injury or advancing age. Your contributions are based on your age
when you enroll for the first time-the younger you are when you first enroll,
the lower your contributions will be. Your contributions will continue to be
based on the date you first enrolled if you have continuous coverage.

Group Legal

What's the Difference?
- If you participate in the group legal plan, you will
have access to local attorneys to discuss wills, divorce, real estate and
other related matters. Certain legal representation will also be covered. You
will pay no additional fees for most services provided.
