Job Details:

- High school diploma or equivalent is required
Under moderate supervision, makes home visits to patients and families. Provides direct services as well as follow-up services with problems such as transportation, housing, nutrition, childcare, etc.

Requirements: High School diploma or equivalent. One year experience working in a community agency, organization or healthcare setting. Must hold a Maryland driver�s license and have a car.
