Patients who have been discharged from a hospital’s acute care services, but still need specialized treatment therapies before going home, may qualify for care in the Subacute Care Unit at Northwest Hospital.

The goal of subacute care is to provide patients in the 24-bed long-term care facility with the specialized care and education needed to function in a home setting upon discharge.

Each patient room in the Subacute Care Unit is decorated in warm, cozy colors and rich fabrics.  Patients recover in an attractively furnished area designed to provide physical comfort during their stay on the unit.   All rooms are private and equipped with a private bathroom and shower.

The unit focuses on rehabilitation for patients following orthopedic surgeries and procedures that require occupational, physical or speech therapy.  Patients are also treated for a range of diagnoses including congestive heart failure, sickle cell disease, and other medical conditions that require additional therapies.

Convenient access to acute hospital services when needed, including diagnostic testing, CT scans, magnetic resonance imaging (MRI) or basic medical procedures, make patient stays as disruptive as possible.

Participation in unit recreational activities that provide comfort and stimulate self-esteem, creativity and emotional independence is encouraged.

Hospice Care
Hospice services are also provided, with assistance from outside hospital affiliates, for those patients requiring compassionate end-of-life care. Located within the Subacute Care Unit, hospice patients may have visitors around the clock, and arrangements can be made for loved ones to stay overnight in the rooms with patients.
