How do go about obtaining a copy of my birth certificate for myself or someone I know?

All birth certificates are issued by the Bureau of Vital Statistics. Sinai Hospital does not issue birth certificates. You can contact the Bureau of Vital Statistics at www.vitalcheck.com or you can reach Maryland Vital Records at 410-764-3038.



I am coming out of town and will need a place to stay. Do you offer any accommodations or lodging?

Designed with the patient in mind, several guest suites are available at The Hackerman-Patz House at Sinai. How do I obtain a copy of my medical records?

To obtain copies of your medical records, you may  print the "Authorization for Release of Medical Information" PDF form and mail it directly to the facility or specific office you are requesting records from. We do not accept requests for medical records by e-mail over the Internet. Please allow 7-10 business days for your records to be copied.   Please complete the authorization form, include all information requested and be specific as to dates of service or the types of reports needed.  If the record is being released directly to your private physician or another health care facility,  there is no charge associated with copying your records.  If you have a follow-up appointment, be sure to include the date of your appointment. However, if you request copies to be released to yourself or others, there are photocopying charges.  (State of Maryland fees for copying are:  base fee $19.09, plus per page fee of $0.63 and the actual cost of shipping.)
