Admission Criteria
- Must have a brain injury diagnosis- Traumatic brain injury, stroke, aneurysm
clipping, or other acquired brain injury
- Must be medically stable and have physician�s
order
- Must have reliable two way communication
- Ability to benefit from and tolerate group setting
- Motivation to improve and/or have work, school,
community re-entry goals
- Ability to modify behavior with repetition and cueing
- Must have reliable transportation- family, MTA
mobility, Medical Assistance transportation, other.*
- Must be able to complete basic Activities of Daily
Living with minimal assistance
- Must be continent during the daytime
- Must have viable funding

Referral Process
Referrals to our RETURN! Brain Injury Day Program come from physicians, other health care professionals, community agencies, family members, friends, and survivors. Once the referral is made, our Intake Social Worker or Program Coordinator assesses if the program would be beneficial, verify funding source, obtain necessary documentation, clarify transportation, and finalize admission date. For the initial visit, we ideally would request family members accompany the clients, so families will have the opportunity to meet with the Program Coordinator and Social Worker.
