Request Forms:
All specimens must be accompanied by a properly prepared and completed Surgical Pathology Consultation Request Form.
Inpatient: Patient information stamp, specimen source, clinical information
Outpatient/Outside Patient: Patient information must include name, address, date of birth, social security number, physician name, specimen source, pertinent clinical information and billing/insurance information.
Fixation:
The fixative of choice for all routine specimens is 10% buffered formalin. Some specimens require special fixation, special processing or must be processed fresh. Any questions concerning fixation or the handling of specimens should be directed to 601 - 5l03.
Biopsies Requiring Special Handling:
Kidney, Breast, Heart, Lymph Node, Muscle, Nerve, lmmunofluorescence, Tumor Hormone Receptors and Drug Resistance Assays, Cultures and Microbiology Studies, Electron Microscopy, Cytogenetics, Molecular Pathology, Frozen Section, lntraoperative Consultations and Gross Examination
