Insurance Plans Accepted
Northwest, Carroll, Sinai, Levindale and Grace Medical Center all participate in a wide range of health plans and accept most major insurance providers. We accept all private pay patients, as well as the insurance plans listed below. Before scheduling an appointment or seeking treatment, please contact your insurance provider to verify full coverage.
When necessary, we will verify your insurance coverage and obtain authorization from your insurance carrier. Please note that at times your insurance provider may not pay for all services if your benefit plan requires out-of-pocket expenses to be paid, such as deductibles, co-payments and co-insurance. Please note that if you are required by your insurance plan to seek services such as lab work, radiology, physical therapy and/or preoperative exams at specific locations other than the hospital at which you receive services, you may be responsible for charges for services provided outside of that network.
